Adult ADHD — in-depth orientation questionnaire

After a screen like the ASRS, this orientation questionnaire digs into today’s attention and impulsivity, traces before age 12, continuity and impact — without ever replacing a clinician’s DIVA-5 interview.

Take the test now

Ideally done after the ASRS test. We’ll dig beyond screening: your current attention and impulsivity, their childhood traces, whether they have followed you over time, their real impact, and what could also explain them. Answer frankly — it’s not about “passing”, but about seeing clearly.

~7 min. This is an orientation, not a diagnosis — the result is meant to share with a professional. Your answers stay on your device.

MoodTrakr orientation questionnaire (2026), built from ICD-11 (WHO) and DSM-5-TR (APA), with the NICE NG87 and CDC framework. Not clinically validated. DIVA-5® belongs to the DIVA Foundation: none of its content is reproduced.

What is this test for?

This questionnaire helps you prepare for an appointment: it puts words to current inattention and hyperactivity-impulsivity (including inner restlessness), asks about examples before age 12, continuity over time, impact across several areas of life, compensation strategies, and what else could explain them (sleep, anxiety, depression, substances, burnout). It is an orientation, not a verdict.

Who is it for?

For adults who recognise difficulties with attention and organisation and want to see more clearly before talking to a professional. ADHD often coexists with mood and anxiety disorders.

How does it work?

You answer 25 statements (an agreement scale and yes/no). The result makes no diagnosis: it indicates whether current signals, childhood, continuity and impact converge enough to make an appointment relevant, still need clarification, or seem unlikely today — then points you toward the ASRS screen, the executive-function profile or mood and anxiety tests depending on what stands out. Everything is computed in your browser.

The science behind this test

The areas explored draw on adult ADHD criteria (ICD-11 6A05, DSM-5-TR) and the international diagnostic framework: a diagnosis is never made on a scale alone (NICE NG87) — it requires a thorough clinical interview, a developmental timeline, functional impact and several sources. The semi-structured DIVA-5 interview (DIVA Foundation) is the reference tool in adults — this questionnaire neither reproduces nor replaces it.

Full transparency

A MoodTrakr orientation questionnaire, built from DSM-5-TR / ICD-11 criteria and the cited guidance. It does not replace the tools used by professionals (DIVA-5, psychiatric evaluation) and makes no diagnosis. No sensitivity, specificity or probability can be honestly displayed for an unvalidated questionnaire.

  • MoodTrakr orientation questionnaire, not validated: it prepares a consultation, it neither screens nor diagnoses.
  • An ADHD diagnosis is never made on a scale or questionnaire alone (NICE NG87): it requires an in-depth clinical interview and multiple sources.
  • Sleep, anxiety, depression, substances or burnout can mimic ADHD — only an assessment can tell the difference.

The reference tests used by professionals

This questionnaire replaces none of the tools used in consultation. Here are the ones professionals actually use:

  • DIVA-5 (DIVA Foundation)the semi-structured diagnostic interview for adult ADHD, conducted by a clinician — this questionnaire neither replaces it nor reuses its items
  • ASRS v1.1 (WHO)the short validated screening, to do beforehand — it is also in MoodTrakr
  • Full psychiatric assessmentthe diagnosis is clinical and multi-source, and rules out other causes (sleep, anxiety, depression, substances)

Frequently asked questions

Does this test replace the DIVA-5?

No. The DIVA-5 is a semi-structured diagnostic interview conducted by a clinician. This orientation questionnaire neither reproduces nor replaces it: it only helps you prepare for an appointment.

Does a “many elements” result mean I have ADHD?

No. No questionnaire diagnoses ADHD. A marked result simply means a professional opinion would be consistent: only a psychiatrist can conclude, after a full evaluation.

Why questions about sleep, anxiety or alcohol?

Because these factors can mimic ADHD. Flagging them “disqualifies” nothing: telling them apart is exactly the job of an evaluation.

Are my answers private?

Yes. Everything is computed in your browser; nothing is sent to a server. You can then save your result, encrypted, in the app if you wish.
